Clinical Liaison, LVN or MSW

The Clinical Liaison position is responsible for creating and maintaining positive customer relationships in order to increase referrals for Home Health, Hospice, Home Infusion, PACE, Transitional Care Unit and other services within assigned accounts.

Required Qualifications: 

• Education sufficient for clinical licensure for discipline (e.g., LPN LVN, Registered Dietitian) or,
• Bachelor's Degree - Social Work. 
• Current state professional license or certification appropriate to discipline upon hire. 
• 3 years - Experience in clinical discipline.

Preferred Qualifications: 

• Master's Degree - Social Work.
• Bachelor's Degree - Applicable clinical discipline (e.g., Respiratory Therapy).
• Liaison, marketing or sales experience.
• Health care industry experience.
• Documented successful sales account medical management experience.
• Epic EMR experience.
