A Clinical Liaison for home health and hospice serves as the primary bridge between acute care facilities (like hospitals and skilled nursing facilities) and the home care or hospice agency. This role combines clinical expertise with relationship management to ensure that patients transitioning from a hospital to home-based care do so safely and without interruption in their treatment.

Key Responsibilities

  • Patient Assessments: Conducting clinical evaluations of referred patients to determine if they meet the specific eligibility criteria for home health or hospice services.
  • Care Coordination: Collaborating with hospital case managers, discharge planners, and physicians to facilitate a seamless "handoff".
  • Relationship Building: Maintaining professional partnerships with referral sources (doctors and social workers) to ensure the agency remains a top choice for patient placement.
  • Patient Advocacy & Education: Meeting with patients and their families to explain the benefits of home care or hospice, answer questions about insurance coverage, and manage expectations for the transition.
  • Marketing & Growth: Tracking referral activity and implementing outreach strategies to help grow the agency's patient census.
