The Clinical Liaison is responsible for Westminster Canterbury at Home, LLC (WCAH) serves to facilitate the admission of a hospice or home health potential client between their referring physician, the WCAH outreach coordinators, family, and WCAH. May provide in-service education sessions and do public speaking on hospice, home health, and related topics.
 
Responsibilities:
 
The Clinical Liaison meets with potential patients at home, in the hospital or elsewhere to educate them on the eligibility requirements and benefits of home health or hospice.
  • Meets with potential patients' families to help them through their loved ones decision making process.
  • Obtains orders and collects pertinent medical information from the physician, nursing home, and/or assisted living facility.
  • Prepares the WCAH clinical team for the new patient and sets up the format admission.
  • Attends hospice IDG meetings and home health clinical meetings.
  • Responds within 24-hours to all hospice inquiries.
  • Speaks to community groups, referral sources and other audiences on topics such as hospice, home health, advance directives, and other health related issues.
  • Works closely with the hospice chaplain and the outreach coordinators to respond Potential referrals.
  • Teams with the chaplain to manage the Bridge program.
  • Provides nursing visits as time allows.
  • All other duties as assigned.
Qualifications:
  • RN or LPN license required.
  • Minimum of 5 years home health or hospice experience required.
  • Previous Clinical Liaison experience is a plus.
  • Excellent Communication Skills
  • Valid Drivers License.
