Freedom Village at Brandywine is recruiting for a hospitality focused Clinical Liaison to join our team! We are a small skilled nursing facility with only 49 beds. The Clinical Liaison serves as the external face of the senior living community’s health center, responsible for building and maintaining strong referral relationships with hospitals, physicians, discharge planners, and other healthcare providers. Operating primarily in the field, the Clinical Liaison evaluates potential patients, facilitates the referral and intake process, and independently makes clinical decisions regarding patient admissions. This role is vital to driving census goals and ensuring appropriate and timely admissions to the community's post-acute or long-term care services.

The essential job functions of a Clinical Liaison:

• Serve as the primary liaison between hospitals, physicians, and the senior living community’s health center.

• Conduct in-person clinical evaluations of potential residents/patients in hospital or other referral settings.

• Collaborate with the LCS Regional Health Care Marketing and Sales Director to ensure consistency in LCS standards and best practices and adhere to SalesFIRST Standards.

• Assess medical necessity, clinical appropriateness, and payer eligibility to independently approve admissions.

• Develop and maintain strong relationships with referral sources, including case managers, discharge planners, and physicians.

• Provide education and information about the community’s healthcare services and care capabilities to referral sources and families.

• Coordinate the admission process from referral through arrival, ensuring seamless communication with internal staff.

• Collaborate with nursing, therapy, and administrative teams to prepare for incoming admissions.

• Maintain detailed and timely documentation in CRM and EMR systems.

• Track referral and admission data and contribute to marketing and outreach strategy.

• Represent the community at hospital-based events, health fairs, and professional conferences.

• Perform incidental housekeeping and maintenance tasks as may arise during the course of regular duties, in order to maintain a clean, safe, pleasant environment for residents, visitors and staff.

• Perform all other related duties as assigned in an effective, timely and professional manner.

EXPERIENCE & EDUCATION:

• Graduate of accredited school of nursing with a current license.

• Minimum 3–5 years of clinical experience in acute care, post-acute care, or senior living settings.

• Prior experience in a liaison, admissions, or business development role preferred.

• Proficiency with CRM and EMR platforms; experience with PointClickCare is a plus.

LICENSE/CERTIFICATION OR OTHER SPECIAL REQUIREMENTS:

• Licensed Clinical Professional (RN, LPN, LVN, or other healthcare licensure depending on state regulations) preferred. Candidates with case management experience or a Master of Social Work (MSW) will also be considered.

• Must possess current and valid driver’s license consistent with the requirements determined by size of vehicle and laws of the state in which the driver is licensed

• Must possess personal driving record consistent with the requirements of the Driver Selection Policy

• Reliable transportation and willingness to travel daily to area hospitals and referral sources.

• Doctor’s statement verifying free from communicable disease, if applicable in your state.

QUALIFICATIONS:

• Strong clinical assessment and decision-making skills.

• Excellent communication and interpersonal skills, with the ability to collaborate effectively with staff at all levels and maintain strong relationships with ACO partners.

• Experience coordinating with insurance providers and navigating coverage requirements.

• Self-motivated and capable of working independently in the field.

• Proficiency in the English language enabling the employee to read, write, comprehend and communicate simple instructions, correspondence, memos, etc.

• Ability and willingness to utilize computer software system to track activities and other pertinent information

• Ability to multi-task, balance team and individual responsibilities, and respond promptly to resident and visitor needs

• Ability to utilize Microsoft Office Suite products

• Ability to prepare accurate and timely reports to meet required deadlines

• Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions

• Ability to deal with problems involving few concrete variables in standardized situations

• Must demonstrate excellent telephone communication skills.

• Multi-task oriented

• Willingness to be available for any/all emergencies regarding the community. 

• Experience working in long term care is preferred.
