1. Collaborates with Director of Quality and Performance Improvement to implement effective ongoing educational programs:
2. Ensures reliability of data used to measure patient outcomes and support care decisions.
3. Actively participates in appropriate Agency committees and works with clinicians across all shifts and sites.
Ancillary Duties:
1. Performs other duties as necessary to support the Mission and Philosophy of the Agency.
Knowledge, Skills and Requirements: Knowledge of state, federal and accreditation regulations governing the delivery of home health and hospice care. Excellent oral and written communication skills. Capable of implementing agency goals and direction. Knowledge of Community Health, Home Care Principles and agency Policies and Procedures.
Education: Associates degree in Nursing from an NLN approved school required. Bachelor’s degree in Health Care, or Nursing preferred.
Experience\: One (1) years full-time home health care experience preferred. Formal and informed experience teaching.
Certificate/License: Possession of a current license to practice as a registered nurse in the State of Connecticut. Possession of an individual malpractice insurance policy. OASIS Certification required (COS-C).

The Greater Manchester Region has approximately 2,500 employees. It includes Manchester Memorial Hospital, a 249-bed community hospital, Rockville General, a campus of Manchester Memorial Hospital, a 102-bed facility, a large multispecialty provider group and visiting nurse services. The Greater Manchester Region serves a region of 300,000 people in 19 towns.
General Summary of Duties: Responsible for education, orientation, training, and development of Agency staff.
Supervision Received: Reports to Director of Quality and Performance Improvement.
Supervision Exercised: None
Typical Physical Demands: Requires prolonged sitting, requires some standing, bending or stooping. Requires eye-hand coordination and manual dexterity. Requires the ability to lift a minimum of 30 pounds. Requires the ability to ascend/descend stairs. Requires the ability to distinguish letters, numbers and symbols. Requires corrected vision and hearing to normal range. Requires use of telephone, office equipment, copier, and computer.
Typical Work Conditions: Work is performed in an office and patient care environment. Contact with agency management, clinical, and financial staff and community.
