Join Community
Community Health Network was created by our neighbors, for our neighbors. Over 60 years later, “community” is still the heart of our organization. It means providing our neighbors with the best care possible, backed by state-of-the-art technology. It means getting involved in the communities we serve through volunteer opportunities and benefits initiatives. It means ensuring our dedicated caregivers can learn and grow to stay at the top of their fields and to better serve our patients. And above all, it means exceptional care, simply delivered — and we couldn’t do it without you.
Make a Difference
The Clinical Documentation Specialist nurse facilitates modifications to clinical documentation through interaction with physicians and members of the interdisciplinary team (attending physicians, allied health practitioners, bedside nursing, consultants). Evaluates, facilitates, and obtains appropriate provider documentation for any clinical conditions or procedures to support the appropriate severity of illness, expected risk of mortality, and complexity of care of the patient. The goal is to provide each patient with an accurate and specific medical record that clearly communicates the assessment, treatment rendered (sometimes requiring consult orders) and outcome of that treatment, and to provide information to any subsequent providers or caregivers at the next site of care.
