Title: Clinical Dietitian/Diabetes Educator – Endocrinology

POSITION SUMMARY:
The Clinical Dietitian /Diabetes Educator provides expert nutrition assessment, education, counseling, and care coordination for patients with endocrine disorders such as diabetes, growth disorders, and other metabolic conditions. This role involves administering diabetes education, including medication management, self-monitoring, and lifestyle changes; planning and developing individualized medical nutrition therapy; collaborating with interdisciplinary teams, supporting patient and family education, and participating in quality improvement and community outreach initiatives to promote optimal health outcomes. He/she supports the corporation’s mission and coordinates nutritional care with other health care providers, the client, the client’s family/caregiver, and the community.
MINIMAL QUALIFICATIONS:
- Certification as a Diabetes Care and Education Specialist from CBDCE
- Registered Dietitian status with the Commission on Dietetic Registration (CDR) of the ADA, bachelor’s degree with a major course of study in human nutrition, food and nutrition, dietetics, or food management.
- Knowledge of pediatric endocrine disorders, diabetes management, and relevant clinical guidelines.
- Experience in pediatric endocrinology is preferred.
- Completion of the American Dietetic Association (ADA) approved Standards of Education.
- Current Florida licensed dietitian status with the Dietetics and Nutrition Practice Council
- Insurability (malpractice).
- BLS certification
- Bilingual proficiency in English and Spanish, both verbal and written, preferred.
Special Skills:
- Minimum two years of work experience as a Registered Dietitian.

- Excellent customer service, interpersonal, and communication skills across various settings.
- Ability to effectively deliver nutrition and wellness education via video, written materials, and in-person presentations.
- Knowledge of general and therapeutic nutrition, with strong analytical skills to assess nutritional status and client motivation.
- Experience counseling pediatric clients and families, both verbally and in writing.
- Strong organizational skills with the ability to manage workload and meet deadlines.
- Ability to represent Central Florida Health Care, Inc. positively in community outreach and wellness initiatives.
RESPONSIBILITIES AND PERFORMANCE EXPECTATIONS include, but are not limited to, the following:
- Be familiar with CFHC’s program philosophies, goals, and objectives. Be cognizant of and comply with all CFHC’s policies and procedures, as well as state and federal regulations.
- Maintain timely communication with healthcare providers regarding patient progress, issues, and referrals, while upholding productivity standards through effective time management and task prioritization.
- Maintain accurate and complete documentation of all clinical activities, assessments, care plans, educational sessions, and interventions in the electronic health record, to ensure proper record keeping for workload accounting, scheduling, billing, and compliance with privacy and safety standards.
- Participate in staff outreach and off-site health care programs requiring Nutritional services as requested by the DON, DAPP, CMO or CEO. Represent CFHC and serve as a resource for staff and community education initiatives, including health fairs/events, school screenings, and community programs by providing diabetes and nutrition education, screenings, and outreach.
- Support performance improvement by developing nutritional care indicators, evaluation criteria, collecting and analyzing continuous quality improvement data, monitoring outcomes, and implementing corrective actions.
- Report as needed to the CMO or CMO delegate any pertinent information or situations that impact patient care or CFHC liability.
- Attend and participate in mandatory CFHC meetings (Center Specific meetings, corporate meetings, and other meetings as designated by the CMO or CEO).
- Demonstrate ability to work cooperatively with other members of the clinical team. Be supportive of coworkers.
- Conduct comprehensive nutritional assessments for pediatric patients at nutritional risk, including those with endocrine disorders like diabetes and growth issues.
- Provide personalized nutrition counseling and develop, implement, and monitor individualized nutrition care plans for pediatric patients with endocrine conditions, addressing topics such as carbohydrate counting, meal planning, healthy eating habits, and lifestyle modifications tailored to the patient’s cultural, religious, and educational needs. Interpret relevant medical data, including lab values and medical histories, to inform nutritional interventions.
- Assess educational needs, evaluate patient and family understanding, and modify teaching approaches as necessary. Adjust care plans based on patient progress through integrated care and collaboration with physicians, nurses, social workers, and other healthcare professionals to ensure comprehensive management of pediatric endocrine conditions.
- Develop and deliver age-appropriate, culturally sensitive educational materials, including handouts, workshops, and online resources.
- Educate healthcare team members about pediatric endocrine disorders and nutritional management best practices, incorporating current guidelines from organizations like ADA, AAP, and others.
- Participate in the development, evaluation, and continuous improvement of nutrition education programs, ensuring content is current, evidence-based, and aligns with accreditation standards.
- Support research and quality initiatives related to pediatric endocrine nutrition care.
- Collaborate with community organizations and schools to promote health and nutrition awareness.
- Support compliance with all applicable safety, infection control, HIPAA, Joint Commission, ADA, and regulatory standards.
- Maintain a neat and professional appearance at all times.
- Carry out any other activities deemed appropriate by the CMO or CEO.
