What it's like to be a Clinic Health Coach at Clinic Health Coach

As a Full-Time Clinic Health Coach at Guttenberg Municipal Hospital & Clinics, you will play a pivotal role in overseeing a comprehensive database of targeted patient populations. Your mission will involve identifying patients needing chronic disease care management and actively participating in quality improvement initiatives. You will create, communicate, and coordinate essential tools aimed at enhancing patient outcomes, while also providing education and training to those with chronic conditions. Engaging closely with the healthcare team, you will monitor patients’ progress, set achievable short- and long-term goals, and collaboratively develop individualized care plans.

Your ability to clarify patient needs and foster self-efficacy will empower individuals to overcome environmental obstacles and adopt healthier behaviors. With a focus on accountability and non-judgmental communication, you will support and motivate patients throughout their journey, conducting initial consultations and evaluations to ensure they receive the guidance necessary for positive change.

What we're looking for in a Clinic Health Coach

To thrive as a Full-Time Clinic Health Coach at Guttenberg Municipal Hospital & Clinics, candidates must possess a robust set of skills and qualifications. Applicants should be graduates of an accredited LPN or RN program with a minimum of five years of clinical experience, and they must maintain current registration and a valid license with the Iowa Board of Nursing. BLS certification is essential, along with the clinical knowledge required to deliver quality nursing care effectively.

A successful Health Coach must display high levels of empathetic understanding, tact, and kindness towards patients, employing strong communication skills to foster collaboration within the healthcare team. Additionally, completion of a Health Coach training course will further enhance your capability to support patients effectively. Being a team player is vital, as the role demands collaboration in goal-setting and the development of personalized care plans, underscoring the importance of adaptability and a patient-centric approach.

Knowledge and skills required for the position are:

• Graduate of an accredited LPN or RN program and 5 years LPN or RN experience in a clinical setting.
• Must maintain current registration and valid license with the Iowa Board of Nursing. Must be BLS certified.
• Must have clinical knowledge necessary to competently perform quality nursing care duties. Must display a high degree of empathetic understanding tact and kindness towards others have the ability to communicate well and work effectively with others as a team player.
