We are hiring a Certified Patient Navigator to support patients through their care journey in Florida. This is a telecommute / work from home position for candidates who currently reside in Florida and are certified as Patient Navigators in the state. The role requires hands-on navigation experience and strong knowledge of Medicare and Medicaid.

Responsibilities

  • Guide patients through appointments, referrals, and follow-up care.

  • Assist patients with Medicare and Medicaid eligibility, benefits, and coverage questions.

  • Provide clear, supportive patient education and next steps.

  • Maintain accurate and timely patient records and documentation.

  • Communicate with clinics, care teams, and community partners.

  • Identify barriers to care and connect patients to appropriate resources.

  • Ensure HIPAA compliance and protect patient confidentiality.

  • Support patients by phone, email, and telehealth as part of a telecommute workflow.

Required Qualifications

  • Active Florida state certification as a Patient Navigator.

  • At least 1 year of direct experience working as a Patient Navigator.

  • Strong working knowledge of Medicare and Medicaid.

  • High school diploma or GED.

  • Experience using EHR systems and scheduling platforms.

  • Strong communication and organizational skills.

  • Ability to work independently in a home office environment.

  • Must currently reside in Florida and be legally authorized to work in the United States.

Preferred (Not Required)

  • College coursework or a degree in a health-related field.

  • Additional experience in clinics, hospitals, care coordination, or case management.

Additional Information

  • Flexible part-time schedule.

  • Hourly rate based on experience and certification.

  • Level II background check required.

  • For a Florida-certified Patient Navigator role, residency in Florida is the norm and should be required.
