All Coastal Health District County Health Department employees are deemed essential employees in the event of an emergency. As such, you may be called on to participate in an event not limited to but including, staffing a shelter, a Specimen Point of Collection Site (SPOC) and/or Point of Dispensing site (POD). When you are called on to participate, you are expected and it will be a part of your job requirement to participate, whether the emergency occurs during or outside regular work hours.
CAREWare Management: Serving as the database manager for CAREWare, the primary information system used to track Ryan White services.
Data Collection & Entry: Lead the systematic collection and entry of client-level data, including demographics, service utilization, and clinical outcomes.
System Integrity: Maintaining and troubleshooting electronic health records (EHR) systems to ensure data accuracy and security.
Reporting and Compliance
Federal Reporting: Producing and analyzing required reports to HRSA and State for review, such as the Ryan White HIV/AIDS Program Services Report (RSR).
Grant Compliance: Ensuring data supports the monitoring and management of federal and State Ryan White Part B, C and D grant deliverables.
Performance Measures: Help tracking and reporting on clinical quality indicators, such as viral suppression rates.
Quality Improvement: Using data to identify service gaps, monitor HIV-related health outcomes, and evaluate the effectiveness of care programs.
Technical Assistance: Providing data-related training and support to internal staff and external recipients if needed.
Data Validation: Reviewing data for completeness, promptness, and accuracy in accordance with HIPAA as well as program policies and HRSA/State requirements.
Clinical Coordination
Patient Tracking: Assisting with tracking patient engagement, such as reducing no-show rates and monitoring linkage to care. Helping to prepare and submit reports to HRSA and the district leadership regarding program performance indicators and tracking system.
Service and Performance Monitoring:
Collaborating with case managers to update Individual Service Plans (ISPs) and monitor patient acuity. Tracking, monitoring, and reporting on clinical performance measures (e.g., viral suppression, linkage to care, patients visit frequency, ART prescription) to improve client health outcomes.
Associate degree in Business or related field from an accredited college or university and one (1) year of experience related to area of assignment.
Preferred qualifications: Degree in computer science, business administration, public health or another related field.
* Proficiency in CAREWare, VHN or any other Electronic Medical Records (EMR) or database as well as Microsoft office.
* Knowledge of HIV/AIDS care or other infectious diseases and data analytics/management skills.
* Experience with federal grant reporting will be a plus.
Note: An equivalent combination of knowledge, education, job or intern experience, training, or certifications that provides the necessary knowledge and skills to successfully perform the job at the level listed may be substituted year-over-year.
