What will you be doing in your role?
Client Intake and Discovery
- Conduct client intake and assessment.
- Ensure service plan and PSSP are completed as part of Discovery Session.
Client Engagement
- Utilize coaching approach to empower clients and maintain engagement and motivation to achieve goals.
- Help clients connect with valuable resources by advocating with community partners and providing financial assistance as needed.
- Exercise flexibility to serve the needs of the clients and are willing to provide direct services as needed.
Service Planning
- Stay informed about community resources.
- Ensure goals identified on service plan remain the client’s goals while in service.
- Meet with clients regularly to review goals and progress.
- Exercise stewardship in the allocation of funds available to assist clients.
- Hold clients accountable to set goals.
Cross Team Collaboration
- Support volunteers assigned to office.
- Collaborate effectively across multiple internal and external programs to enhance the client experience.
- Travel to other CCFW campuses as needed for team meetings and/or participate in professional development.
Documentation
- Complete all necessary documentation to ensure compliance with funding requirements, licensing requirements and agency quality assurance standards.
- Maintain all records of communication and data collected from clients in online database system.
Are you the next Short Term Case Management Client Navigator?
- Bachelor’s degree in Social Work or related field, OR a combination of 2 years of relevant work experience and education.
- Social work or case management experience in a related field.
- Bilingual in Spanish, required.
- Must have reliable transportation, valid driver’s license, and liability insurance.
- Must have ability to work a web-based database.
- Occasional travel to meetings with community partners and our Fort Worth office as needed.
