JOB SUMMARY:
The Behavioral Health Case Manager within the RENEW Center and partner facilities is responsible for delivering client-centered, strengths-based case management services to individuals with substance use and co-occurring mental health disorders. This role supports individuals in achieving greater stability, independence, and wellness through coordination of care, resource navigation, and recovery-focused support. Under the supervision of the Clinical Director, the Case Manager works collaboratively with clients, families, treatment teams, and community providers to ensure timely referral, linkage, monitoring, advocacy, and follow-up. This position plays a vital role in promoting self-sufficiency and maintaining community tenure, in accordance with OAC Title 450, Chapter 50 and center/agency policy.  
DUTIES AND RESPONSIBILITIES:
  • Conduct and document comprehensive, strengths-based case management assessments as part of the intake, treatment planning, and discharge processes, addressing community functioning, education/employment, medical needs, support systems, legal/financial issues, living environment, and barriers to recovery.
  • Develop individualized Case Management Care Plans in coordination with the clinical team that reflect client needs and goals.
  • Coordinate referral and linkage to community resources including housing, transportation, employment services, education, medical/dental care, and legal support; monitor the overall resource or compliance with various referring entities of individuals served.
  • Advocate on behalf of clients with external agencies such as Department of Human Services, probation/parole, courts, and health providers as appropriate and with proper consent.
  • Monitor progress toward treatment and case management goals, through regular contact and reassessment, and adjust support strategies in collaboration with the individual served.
  • Provide ongoing support, skills-building, and education to promote self-advocacy and empower clients to independently navigate community resources/systems; also includes Psychosocial Rehabilitation services.
  • Participate in crisis diversion activities by providing timely, unscheduled support during immediate, destabilizing situations, either face-to-face or via phone.
  • Follow up with clients who miss appointments for critical services such as mental health care, medication management, medical visits, and/or other supports.
 
  • Respond to client needs in a timely manner (including addressing emergent issues) and maintain regular (at least monthly) contact with clients as indicated by treatment plan.
  • Ensure all case management activities are accurately and thoroughly documented in the client’s record in a timely manner.
  • Serve as the primary case manager for assigned clients, coordinating services to promote continuity of care and reduce duplication of services across providers and support systems.
  • Offer case management services to clients and identified family members upon request; respond to such requests promptly and document all follow-up.
  • Support discharges by coordinating linkage/referral to ongoing resources in the client’s home community and completion of required paperwork; for unplanned discharges, initiate follow-up and document efforts in the record as well.
  • Maintain compliance with all privacy and confidentiality laws, ethical standards, and agency/center policies, including HIPAA guidelines and obtaining written consent before releasing or sharing information unless legally mandated.
  • Maintain Certification status through Oklahoma Department of Mental Health and Substance Abuse Services; adherence to all rules, regulations, and ethical codes set forth by the entity.
  • Completion of Continuing Education in the field to meet certification requirements and for professional development.
  • Perform other duties as assigned.
 
SUPERVISORY RELATIONSHIP:
The Behavioral Health Case Manager works under the direct supervision of the Clinical Director and performs no staff supervision.
 
QUALIFICATIONS:
  • High school diploma or GED required, with at least 36 months of direct, documented experience working with persons who live with mental illness and/or substance abuse issues, OR college credit/degree and combination of experience required per Certification standards.
  • Bachelor’s or Master’s degree in Psychology, Sociology, or related field of study preferred.
  • Minimum two (2) years’ experience providing behavioral health case management services.
  • Completion of Behavioral Health Case Management II training and certification through Oklahoma Department of Mental Health and Substance Abuse Services.
  • In-depth knowledge of community-based behavioral health systems, recovery supports, and relevant state regulations, as well as knowledge and/or experience related to the provision of services within the criminal justice population and corrections systems.
  • Experience working with individuals impacted by substance use disorders, co-occurring mental health conditions, or similar populations preferred.
  • Strong communication, organizational, and advocacy skills.
  • Ability to build rapport with clients and maintain professional boundaries.
  • Proficiency in basic computer applications, including electronic documentation systems.
  • Demonstrated integrity, dependability, and commitment to client-centered care.
 
 
MENTAL AND PHYSICAL DEMANDS/ENVIRONMENTAL FACTORS:
  • Constantly must be able to sit/stand in a stationary position 50% of the time, stoop, bend; walk; talk; hear; use hands to grip, type; reach with hands and arms, occasionally required to stand or walk on uneven surfaces; climb stairs
  • Frequently must be able to move about inside the office to access file cabinets, office machinery, etc.
  • Frequently must operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer
  • Frequently must communicate with staff and others who have inquiries about the center/programs; must be able to exchange accurate information in these situations
  • Occasionally must move/carry items weighing up to 30 pounds
