We provide healthcare services including medical, dental, mental healthcare, WIC, and community outreach services to anyone in our community who may be living in poverty or who may be underserved. N.E.W. Community Clinic has three dental clinics, two medical clinics, a WIC location, and a behavior health clinic in Green Bay, and we are seeking an experienced and empathetic Behavioral Health Case Manager to join our enthusiastic team as we expanding our clinic.

Position Summary:

The Behavioral health case manager is responsible for coordinating care to individuals and families receiving services through NEWCC. This includes assessing barriers and implementing strategies to produce quality outcomes for health and wellness as a collaborative team member.

Essential Functions of the Job:

  • Conduct case management for the behavioral health department, shelters, and other clinics as needed in collaboration with medical, dental, and WIC departments.
  • Assess client's healthcare and socioeconomic needs. Make referrals for medical, dental, health benefits, and behavioral health care in collaboration with interdisciplinary team as needed.
  • Develop a plan of care to address identified client needs in collaboration with the client and the care team.
  • Assist clients in addressing identified barriers and challenges to meeting needs. Although the primary focus of the program is behavioral healthcare, the Case Manager also assists clients in accessing other services such medical, dental, WIC, housing, employment, day care, transportation, insurance, Social Security, disability benefits, community resources, etc.
  • Coordinate, manage, and document medical, dental, vision, behavioral health, and specialty referrals, including scheduling appointments, addressing insurance issues, ensuring quality care, and advocating to reduce barriers to accessing care.
  • Assist clients in completing either online or via phone Foodshare, BadgerCare, Medicaid, Family Planning Waiver, Express Enrollment insurance applications.
  • Provide advocacy for the client to facilitate access to healthcare and other services.
  • Crisis intervention as needed, including suicide assessment, evaluation, triage, and higher level of care referrals. Provide referrals and coordination of mental health services, including fulfilling records requests, requesting external records, and facilitating admission to other agencies.
  • Conduct pre-screening assessments for behavioral health care services.
  • Coordinate and follow through with applications to pharmaceutical companies for medications needed by clients.
  • Assist with appointment reminders, care coordination, and transportation to decrease barriers to client attendance.

Qualifications and Education Requirements

  • Education: Bachelor's degree in social work or related field.
  • Experience: Previous case management or relevant experience preferred. Basic knowledge of case and utilization management practice and methodologies and state and federal healthcare regulations and accreditation required. Knowledge of the community, community organizations, and community resources. Able to use computer, printer, and software programs necessary to the position, e.g., Microsoft Office Suite, Outlook, electronic medical record.
  • Expertise: Ability to provide culturally competent services to a variety of clients in a direct responsive manner. Willingness to confront traditional barriers in respect to servicing patients with various medical and mental health diagnoses. Strong interactive "team" skills within a primary medical care setting requiring minimal supervision. Excellent interpersonal skills and empathy toward patients. Able to develop teaching and counseling sessions, as needed.
  • Language: Comprehend and use basic language either written or spoken to communicate information and ideals. Fluent in the Spanish language desirable.
